Incapacitated or developmentally disabled w/o advance directive, or designated surrogate, health care decisions may be made:

• Court appointed guardian/guardian advocate
• Patient’s spouse
• Adult child of Patient (majority if more than one)
• Parent of the Patient
• Adult sibling of the Patient (majority if more than one)
• Adult relative of Patient (special provisions)
• Close friend of Patient
• Clinical Social Worker (special provisions)
